The purpose of this form is to provide Polk County citizens with a process for notifying county officials about the location of Hurricane Helene storm debris and requesting its removal from public or private roads within Polk County, NC.

To improve the process of having storm debris removed by the USACE contractor, complete the debris removal request form as soon as possible. This form helps the contractors know where debris is located throughout the county and helps them make a plan to speed up the removal process.

While submitting a form doesn’t guarantee a day or time for pickup, it gets your road on the list to be removed during their contract.

We do not know when the contractors will be in specific areas. Locations and the amount of debris requested for pick up will determine how often they will be able to go to one area or on one road. The quicker your debris is ready for removal, the more likely it will be picked up.

Be sure your debris is sorted and placed in manageable piles within ten feet of the road and that the logs are no longer than 15 feet.
